Catastrophic OneShare Plans

The Catastrophic OneShare option is the lowest priced OneShare plan. The Catastrophic plan has only four age ranges: 18-39, 40-49, 50-59, 60-64. In contrast, the Classic and Complete programs cover five age ranges.

For injury and illness, the Catastrophic plan has an ISA that is equal to the Maximum Out-Of-Pocket (Max OOP). At the time of enrollment, members are required to choose either the $5,000 or $10,000 ISA.

Diagnostic, X-Ray and Labs

This category is subject to ISA. Once a member has met the ISA, they become 100% eligible up to a maximum limit per incident. Catastrophic coverage has three tiers for members to choose from during enrollment. The tier you choose will determine your maximum limit per incident, which can be $150,000, $250,000 or $500,000.

Specialist Coverage

With the Catastrophic OneShare plan, there is unlimited specialist coverage. Members are only required to pay a $75 visit fee each time they see a specialist, which goes towards the ISA. After the ISA has been met, specialist coverage becomes free throughout that program year.

A referral fee is not required for specialist coverage. However, the coverage only applies to hospitalization or out-patient surgery. Some examples of specialist coverage include surgery after a heart attack or therapy for a person who has had a stroke.

Telemedicine is 100% eligible in the Catastrophic OneShare plan and is offered through Dial Care. Members get 24/7/365 coverage with a $0 consultation fee. Telemedicine covers conditions like flu, asthma, allergies and migraines.

Emergency Room

All members of the Catastrophic OneShare plan pay a $300 visit fee for emergency room services.

With the Catastrophic OneShare plan, there is coverage for in-patient/out-patient surgery with a two-month waiting period from the effective date. Surgery is 100% eligible for sharing after the ISA has been met up to the maximum limit per incident.

Hospitalization is 100% eligible for sharing after the ISA has been met up to the maximum limit per incident.

Lifetime Maximum

The lifetime maximum will depend on the tier you choose during enrollment. There are three tiers available on the Catastrophic plan, with the first one having a $300,000 lifetime max, the second a $500,000 lifetime max and the third a $1,000,000 lifetime max. For each tier, the lifetime max is automatically twice the amount of max limit per incident, which is $150,000, $250,000 and $500,000 for the three tiers, respectively.

The Catastrophic program can be an excellent program for a recent college graduate, young individuals who are aging off their parents’ plan or a newly married couple.
